崇益德健身房(暑期)會員申請
一、會員申請：自2026年6月1日至6月12日16:00截止
二、適用對象：本校教職員生、校際交流外校生及短期交流外校師生
三、暑期週期：自2026年6月29日至8月30日
四、健身房開放時段
(一)2026暑假期間：週一至週日8:00-17:00；18:00-22:00。
(二)健身房清場休息：17:00-18:00
五、暑期/短期會員收費標準：
1.暑期：NT$400
2.短期外籍師生二個月：NT$400
六、進出健身房時，需使用職員證/學生證刷卡入內
需事先至住宿組，辦理卡片驗證會員資格後方可使用，證件限本人使用，若發現冒用將取消使用權。
※申請流程：
填寫申請單→繳費(郵局/出納組)→攜帶申請表及繳費收據至住宿組辦理

I. Application Period: From June 1, 2026, to June 12, 2026, at 16:00.
II. Eligibility:
University faculty, staff, and
students; inter-collegiate exchange students; and short-term international
visiting faculty and students.
III. III. Summer Membership Validity Period：From June 29, 2026, to August 30, 2026.
IV. Gym Opening Hours:
1. Summer Vacation : Monday to
Sunday, 08:00–17:00 and 18:00–22:00.
2. Maintenance & Cleaning Break: 17:00–18:00 daily.
V. Membership Fees：
1. Summer vacation (NT$400)
2. Short-term
International Faculty & Students (2-Month Pass)：NT$400
VI. Please swipe your Staff or Student ID card to enter the gym.
Members must verify their membership at the Student
Housing Section before the card can be activated for gym access. ID cards are
non-transferable. Any misuse or fraudulent use (such as lending your card to
others) will result in the immediate revocation of usage rights.
Fill out the application form → Pay fee (Post Office / Cashier Section) → Bring the application form and payment receipt to the Student Housing Sectuon for processing.
