為利本校後續健康檢查作業之順利安排，敬請依序完成下列表單填寫，感謝您的配合。
To facilitate the smooth arrangement of subsequent health examination procedures at our university, please complete the following forms in order. Thank you for your cooperation.
說明
(1). 基隆、林口校區：健康檢查日期 8/25-8/31，可登記健康檢查日期；未填寫者，將由環安室統一安排健檢日期。
(2). 嘉義校區：※必填※ 健康檢查日期 8/10-8/14 ，務必填單，登記健康檢查日期。
(3). 高雄校區： 健康檢查日期 8/7-8/28，*無*提供表單預約，最晚於健檢日前一週 電聯高雄長庚醫院 07-7317123 分機 3696 或 2168 預約健康檢查。

(2) Chiayi Campus: *Required* Examination dates: 8/10–8/14. Completion of this form is mandatory.
(3) Kaohsiung Campus: Examination dates: 8/7–8/28. Form-based registration is not available. Please call Kaohsiung Chang Gung Memorial Hospital at 07-7317123, ext. 3696 or 2168 to schedule your health exam at least one week in advance.
