長庚大學新進人員體格檢查New Employee Physical Examination
一、依據「職業安全衛生法」第20條及「勞工健康保護規則」第10條規定辦理,新進員工有接受體格檢查之義務。
二、新進人員體格檢查說明:
(一) 體格檢查前:
1. 填寫基本資料 https://forms.gle/Hsme8F3fiKGEYFaE8
2. 下載並列印體檢須知單 (如附件)
(二) 體格檢查:
1. 自行至中央衛生主管機關認可「勞工體格及健康檢查認可醫療機構」完成。
2. 體格檢查費用全額由教職員工自付。
3. 體格檢查需包含以下項目:(建議可攜帶體檢須知前往醫院告知需檢查項目)
※若為游離輻射、甲醛作業等特殊健康危害作業,請詳見附表十,依法規規定進行特殊作業體格檢查。
(1)作業經歷、既往病史、生活習慣及自覺症狀之調查。(勞工一般體格及健康檢查紀錄表)。
(2)身高、體重、腰圍、視力、辨色力 、聽力、血壓與身體各系統或部位 之身體檢查及問診。
(3)胸部X光(大片)攝影檢查。
(4)尿蛋白及尿潛血之檢查。
(5)血色素及白血球數檢查。
(6)血糖、血清丙胺酸轉胺酶(ALT)、 肌酸酐(creatinine)、膽固醇、三 酸甘油酯、高密度脂蛋白膽固醇之檢查。
4. 若能提出符合本校規定且未逾到職日前六個月(含)之檢查紀錄(須為認可醫療機構),得免實施已完成之體檢項目。
(三) 體格檢查後:
1. 將完成之體檢報告正本及體檢須知單(簽名)交至環安室,取得收件證明後得至人事室進行報到手續。
2. 體格檢查結果經環安室職醫(護)判斷工作分派合適性,掃描存檔後由校內內部信封寄還本人。
(若超過2週未收到請務必盡速告知,以利查詢)

According to Article 20 of the "Occupational Safety and Health Act" and Article 10 of the "Labor Health Protection Regulations," new employees are required to undergo a physical examination.

Instructions for New Employee Physical Examination:
(1) Before the Examination:

Fill out basic information at https://forms.gle/Hsme8F3fiKGEYFaE8.

Download and print the Physical Examination Notice (attached).
(2) Physical Examination:

Complete the examination at a medical institution certified by the central health authority as an "approved labor health examination institution."

The full cost of the physical examination is to be covered by the employee.

The physical examination should include the following items (it is recommended to bring the Physical Examination Notice to inform the hospital of the required items):
※ For specific health hazard tasks, such as those involving ionizing radiation or formaldehyde, refer to Appendix 10 for the specialized physical examination requirements in compliance with regulations.
(1) Investigation of work history, past medical history, lifestyle habits, and any subjective symptoms (General Labor Physical and Health Examination Record Form).
(2) Examination and inquiry of height, weight, waist circumference, vision, color vision, hearing, blood pressure, and each body system or part.
(3) Chest X-ray (large image) examination.
(4) Urine protein and urine occult blood tests.
(5) Hemoglobin and white blood cell count tests.
(6) Tests for blood sugar, serum alanine aminotransferase (ALT), creatinine, cholesterol, triglycerides, and high-density lipoprotein cholesterol.

If a physical examination report from an approved medical institution meeting the university's requirements, and conducted within six months prior to the start date, is submitted, the employee may be exempted from repeating the completed items.
(3) After the Physical Examination:

Submit the original physical examination report and signed guidelines to the Environmental Health and Safety Office. Obtain a receipt before proceeding to the Personnel Office for onboarding procedures.

The Environmental Health and Safety Office will review the examination results for job suitability with occupational health staff, scan and archive the report, and return it to the employee via internal mail. (If not received within two weeks, please notify the office promptly for follow-up.)
